Required Documents
We require sufficient documentation to (1) demonstrate that you qualify for assistance and (2) ensure the completed application is both accurate and honest.
Valid state-issued driver's license or identity card.
- Please make sure your name, ID number, address, and expiration date are clearly legible. You may redact other information you deem sensitive.
Amazon employee ID or most recent Amazon paystub.
- Please make sure your name and Amazon's name are both clearly legible.
Most recent bank account statements for all adults living in your household.
- Statements must show (1) the name and address of the account holder and (2) all income and deposits made into each account during the previous banking period. You may redact account numbers, balances, and any visible debits.
Required Documentation Based on Type of Assistance
Mortgage assistance
- Submit a copy of your most recent monthly mortgage billing statement. Please make sure it shows your name, property address, vendor contact information, and monthly payment amount.
Rental assistance
- Submit a copy of your lease and a statement from your landlord indicating the balance currently due.
Utility assistance
- Submit a copy of the utility bill for which you are requesting assistance.
Medical bill assistance
- Submit a copy of the medical bill for which you are requesting assitance.
Instructions
When completing the application form, we strongly urge you to have the required documents readily available and to use these documents to complete the application. Errors on the application may result in ineligibility. All documentation must be uploaded at the time of submission.
Amazon Offsets is currently not accepting applications. The application process will open when we have enough donations to begin funding application requests. We encourage you to check back frequently or sign up for our newsletter to be notified when applications will be accepted.
